Connected Care PHR Privacy Policy

Final Privacy Notice

This privacy notice describes the privacy and processing arrangement for the Connected Care Personal Health Record service commissioned by Frimley Health NHS Foundation Trust on behalf of the Connected Care controller organisations and provided by the Connected Care solution providers.   

As of 28/02/2023, the Connected Care Personal Health Record service has been decommissioned and access to the PHR Connected Care app disabled. 

This means that any data you enter into the PHR Connected Care app after this date will not be transferred into Connected Care and will not be processed or viewed by health care professionals.

We advise users of the Connected Care PHR app to delete it from all devices with immediate effect and should users choose not to follow this advice the Connected Care Controllers accept no responsibility for data protection, data security and any other issues arising as a result of continued use of the app.    

We will continue to treat all information that we hold and process within Connected Care in line with our legal responsibilities and recognised best practice standards and processes. 

Data that is held and processed in Connected Care will only be retained as permitted by law and in line with the NHS Records Management Code of Practice 2021.  Data will be archived in the Connected Care System Analytics platform for a period of 8 years before being securely deleted.
