Implementing a hospital at home service to reduce hospital admissions for frail elderly patients in Frimley
A case study profiling how Frimley Health NHS Foundation Trust set up a hospital at home (virtual ward) and urgent community response service to improve the quality of care for elderly frail patients, and reduce unnecessary and lengthy hospital admissions, has just been published by NHS England and NHS Confederation.
Between April 2022 and March 2023, the frailty service saw 1,429 patients being accepted onto the virtual ward. Their experiences and lessons learned are explored in the comprehensive case study hosted on NHS Confederation’s website.
