Connected Care, part of the NHS, is a dedicated team of people supporting your local health and care services to provide you with a joined-up service to meet your needs.
Working together improves the quality of care you receive because healthcare professionals involved in your treatment have access to the most accurate and up-to date information, enabling them to make informed decisions about your care.
We use secure technology to enable those who care for you to view your health and care information when it is necessary:
For example if you visit Frimley Park Hospital Emergency Department your care team can access your Shared Care Record to find out what medications your GP has prescribed.
This also removes the need for you to repeat your story to different clinical staff saving you time and frustration.
In the past we had to wait for you to tell your care professional that you need help. Now, professionals can combine their clinical expertise with the information in your shared care record to proactively offer you help. For example, if you have diabetes and other complex health conditions your GP can provide you with the Remote Monitoring Service, ensuring regular monitoring and proactive care.
To monitor and better plan your care, healthcare professionals can view information in your shared care record from different angles using our System Insights tool. This allows them to understand what actions might be effective and identify areas where you could benefit from additional support. We have used System Insights to identify the benefits of remote monitoring during the COVID-19 pandemic, and we are now using this knowledge to support individuals with various health conditions. In addition, System Insights can help identify issues at a broader level, such as long wait times in the Emergency Department. This enables your GP to dive into the details and identify which of their patients are currently waiting, allowing them to provide more immediate or appropriate care. We are carefully piloting this development and hope to make it more widely available to minimise unnecessary waits in Emergency Departments.
This local work is part of the NHS Public Health Management programme which aims to improve the overall health and wellbeing of the community.
Working closely with healthcare professionals, we ensure that helpful tools are available to you. For example:
- the Docobo app is used by the Remote Monitoring Service for patients to enter information into their Shared Care Record
- GetUBetter is now available for patients with muscle or joint problems
- Sleep support and Healthier Together provide valuable advice and resources.
