Gestational Diabetes new Referral Pathway
Approximately 1 in 2 people who have had Gestational Diabetes will go on to develop Type 2 Diabetes, which can lead to long-term problems that can often be prevented through making simple changes to lifestyle. The Healthier You programme can support you to make small and lasting changes.
How to join the programme?
A history of GDM is the main entry criteria to the Diabetes Prevention Program. You can also register whilst pregnant.
Patients who self-register while pregnant will be contacted 1-month post-partum, if it is not suitable to commence the program at that point, you can go on hold for up to 3 months.

- History of gestational diabetes/NHS number/General Medical Practice Code.
Group sessions are held at local, accessible community venues at varying times, including daytimes, evenings, and weekends. Alternatively, you can take part remotely (via a computer/laptop/mobile) in the comfort of your own home via our group or 1:1 digital platform.
